Your First Visit

We make every effort to ensure that your time with us is a comfortable experience. When you visit Southwest Oral & Implant Surgery, you will be greeted by a member of our experienced and caring staff. You will complete your medical forms and then you will be taken back to meet Dr. Michael Eklund for your initial consultation.

During your oral surgery consultation, Dr. Eklund will explain your diagnosis, answer all your questions, address any fears or apprehensions you may have, and devise a customized treatment for your particular situation. Occasionally, when warranted, surgery can even be performed the same day as your consultation. In most cases, a complex medical history, or a detailed treatment plan, will require a follow-up appointment. At Southwest Oral & Implant Surgery, we deliver modern-day technology with old-fashioned care.

Please help us do our best for you by bringing the following information to your first visit:

  • Your surgical referral slip and any X-rays, if applicable – you may request that your dentist or physician send these directly to us (allow 10 days to arrive at our office), or you can pick them up and bring them with you.
  • A list of any allergies to medicine
  • A list of medications you are presently taking
  • Contact information for your physician(s)
  • New patient medical forms already completed
  • Insurance information

 Important Things to Note: 

  • All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit. Proof of student status must be provided before surgery.
  • A pre-operative consultation and clinical examination is mandatory for patients undergoing IV anesthesia for surgery.
  • If our office has agreed that your procedure will be performed the same day as your initial consultation, do not eat or drink anything six hours before your appointment and arrange to have an adult drive you home.
  • Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, blood pressure medication, aspirin, anticoagulant therapy, etc.).
